How do I create a waiver form?

To create a waiver form:

  • If using the website, go into the speedyWaiver console, click the Forms tab and click the 'Create New Form' button in the top-right of the screen.
  • If using the app, tap the Forms tab at the bottom, then tap the purple '+' icon towards the bottom-right of the screen.

You will be then taken to the speedyWaiver Form Builder. You build the form up by dropping form fields onto the form, for example drop a 'Name' field to capture the customer's name or the 'Phone' field to capture their phone number. To add a field, drag and drop it into place from the 'Form Fields' palette on the left (or press the '+' button if using a mobile phone).

The field types are:

  • Text/Image Display - this is a special type of field that displays text or images to the customer, and doesn't accept any input from them. You can use it for headings, for the body text of the waiver, for images, etc.

  • Name - use this to record a name (generally the customer's name), or you can use it to capture any other person's names. Note that if there are multiple Name fields in the waiver, the first one is used to provide the name displayed in the Submissions screen and for searching waivers.

  • E-mail - use this to record the e-mail address (generally of the customer). Note that if there are multiple E-mail fields in the waiver, the first one is used to provide the name displayed in the Submissions screen and for searching waivers.

  • Address - use to record a postal address (generally of the customer)

  • Phone - use to record a phone number (generally of the customer)

  • Date of Birth - use to record a date of birth (generally of the customer). Note that if there are multiple Data of Birth fields in the waiver, the first one is used to calculate the customer's age in the submission list.

  • Single Line Input - use to capture a line of freeform text from the customer, eg useful for open-ended questions like 'Which town are you from?'

  • Multi Line Input - use to capture multiple lines of freeform text from the customer, eg useful for longer open-ended questions like 'Describe any pre-existing medical conditions?'

  • Number - use to capture a number, for example a customer's weight. This is for purely numeric data, if the input should be a mixed of numbers and letters (eg '32lbs') then use a Single Line Input instead.

  • Checkbox - use to record a yes/no type question, eg 'Are you under 18?'

  • Date - use to record a date, this can be set to only allow future dates or past dates to be input.

  • Multiple choice - use to record a multiple choice, which can be one where the user is only allowed to select one option, or one where the user is allowed to select more than one option.

  • Dropdown - use to record a dropdown entry - for example useful for gender where the options may be 'Male/Female/Other'

  • Signature - use to record a signature

  • Upload Image - a field where the customer can upload an image, either from their computer/phone or from their camera

  • Repeated Group - a repeated group can contain a set of other fields. This could be useful if you are allowing a customer to put in details of a set of guests, for example. In this case the repeated group could contain a Name and E-mail field and customer would be able to fill them out for each guest.

Once you are done then press the 'Finish Form' button and you will be taken to the Form Details screen where you can name the form and save it.
